What does it mean?
P1138 is a Powertrain (engine, transmission, fuel system) fault code. It indicates: HO2S Circuit Malfunction .
Symptoms
- Malfunction indicator lamp on and the affected bank locked in open loop
- Increased fuel consumption and a sooty or sulphur exhaust smell
- Rough idle and slight hesitation under light load
- Occasional stumble that comes and goes with road vibration
- Emissions inspection failure with incomplete monitors
Likely causes
- Open or broken signal wire between the sensor and the PCM (powertrain control module)
- Short to ground or to battery voltage in the signal circuit
- Sensor connector unplugged, partially seated or with a pushed-out terminal
- Failed sensing element that outputs a fixed voltage instead of a varying one
- Corrosion or water inside the connector bridging the signal and ground pins
- Poor sensor ground or a shared ground point loosened by vibration
- Damaged pigtail where the harness passes close to the exhaust or a moving suspension part
How to fix it
- View the sensor voltage in live data: a flat line near 0.45 V usually means the controller sees no real signal
- Unplug the sensor and back probe the harness, checking for the expected bias voltage at the PCM side
- Test the signal wire for continuity end to end and for shorts to ground and to voltage
- Inspect the connector for water, corrosion, spread terminals and melted plastic
- Wiggle test the pigtail and harness while watching the live value for dropouts
- Verify the sensor ground circuit resistance to the module reference
- Replace the sensor once the wiring is proven, then confirm the signal sweeps normally between rich and lean
